Thank you for allowing Southwest Joint Replacement & Sports Medicine Center to care for your medical needs. This letter is to inform you of a change in our practice. Anupam Pradhan, MD will no longer practice at our clinic effective Friday, Mar. 29, 2024, and Southwest Joint Replacement & Sports Medicine Center will close effective Mar. 31, 2024.
During this time of change, your uninterrupted continuity of care is our top priority.
Your medical record will remain on file with Southwest Joint Replacement & Sports Medicine Center. To have a copy of your medical record transferred to a physician of your choosing, please complete the medical record release form and fax to (972) 566-3556. If you need a referral to an orthopedic specialist in your community, please call (972) 566-5564.
As always, you have the right to choose your healthcare provider.
